We are responsible for supporting adults to regain or maintain their skills, and to achieve their outcomes both around the home and in the wider community. To date, this working relationship has involved working extensively with service users with a range of:
• Learning difficulties
• Long Term Health Conditions
• Behavioural issues which present significant challenges
• Mental and psychological health issues
• Issues associated with autistic spectrum disorder
• Physical disabilities: e.g. spinal injury and low mobility
• Other specialist needs.
Person-centred support plans are developed based on the individual’s input, as well as that of professionals including district nurses, GP’s, Occupational Therapists, family-members and advocacy services. By working in partnership with all relevant agencies, such as local charities and community supports, we are able to provide service users with a comprehensive care plan which addresses their specific needs. We work in collaboration with service users, helping them to gain skills and progress towards their outcomes. Service users are given a high level of choice and control over all decision-making processes. For example, they are able to select their own Support Worker from one page profiles describing interests and personality traits.
All support plans are devised in line with the specific needs and desired outcomes of the individual.
Risk assessments are conducted for every service user prior to the commencement of their care plan, with reassessments carried out throughout delivery of the discharge package. Using bespoke plans to support the service user allows us to cater to their individual needs and manage risks in a positive way. This ensures that they are able to support themselves at home and within the community in a way that does not expose them or third parties to unnecessary risks.
All support plans are reviewed regularly to ensure they are up to date and relevant to the individual’s evolving needs. We also keep records of all positive outcomes, including the achievement of person-centred goals, with the aim of promoting continuous improvement. By providing the trust with monthly reports on these achievements we are able to keep it up to date on each individual service user’s progress.
